My job doesn't fit any of your categories. Is that a problem?
No. This page exists for exactly that. Write what the thing is and what it does when you try it, and a local pro who handles that kind of work decides whether it belongs to them.
Do I need to know what kind of contractor to ask for?
You don't, and guessing wrong costs you time. Describe the job. A fence crew and a garage door outfit read the same note, and each one knows on sight whether the work is theirs.
What does it cost to send this in?
Nothing to send it. The price for the work itself is the pro's to give, and they will want to see the dent or the crack before they say a number. Ask each of them what they charge.
Something smells like gas. Should I use this form?
No. Call 911 or your gas utility first, and get out of the house while you do it. This form is for scheduling work, not for an emergency, and there is no set time by which anyone reaches you.
How many pros will call me?
No set number. Who covers your ZIP code and who is taking new work that week decides it, and some weeks that is nobody. Hear out whoever calls, then hire one of them or none.
